Receipts for Revenue and Health Providers

Health Insurance Providers require a signed letter headed statement of session dates attended, along with my accreditation credentials with the IACP and BACP. I provide these monthly at the end of the month upon your request. If there is an outstanding fee from a late cancellation/no show to an appointment, this will be shown on the statement receipt as outstanding fees owed.

Frequency of Sessions

General Information

The therapeutic relationship is unique in that it is a highly personal and at the same time, a professional contractual agreement. Given this, it is important for us to reach a clear understanding about how our relationship will work, and what both parties can expect. This consent form will provide a clear framework for our work together. Feel free to discuss any of this with me. Please read, and you will be required to indicate that you have reviewed this information and agree to it by filling in the checkbox throughout this document.

The Therapeutic Process

You have taken positive step by deciding to seek therapy. The outcome of your treatment depends largely on your willingness to engage in this process, which may, at times, result in considerable discomfort. Remembering unpleasant events and becoming aware of feelings, thoughts and beliefs attached to those events can bring on strong feelings of anger, depression, anxiety, etc. It is important that clients’ feel emotionally stable enough to engage in this process, and with CBT treatment, the willingness and motivation to change. These are necessary conditions when working with CBT. However, if you are not looking for CBT and seek traditional talking therapy, we will work together to help you clarify what it is you want for yourself.

Change/Cancellation of Appointment

Appointments will be confirmed by text the day before as a reminder. Please provide me with at least 24-hours notice in advance to reschedule/cancel an appointment during the same week. I aim to facilitate clients varying availability needs. In the event that I may be required to reschedule your day/time of appointment to a different day/time during that week, I will aim to provide you with at least 24-48 hours notice.


Cancellations

Less than 24 hrs notice or no show for appointments the full fee of €75 will be charged and required to be paid at the time of cancellation, a payment link will be sent. No further appointments will be offered until the late cancellation/no show fee is paid. The late cancellation/no show fee will be waivered if the appointment is rescheduled during the same week (this is dependent upon appointment availability). In the unlikely event that the therapist does not show up on the day of the appointment, you will be offered an alternative appointment free of charge. Genuine cancellations with notice are understandable, however due to high demand for appointments, more than two cancellations within a 4-week period will require future appointments to be paid in advance to secure the appointment. More than three cancellations of appointments within a 4-week period will result in no further appointments being offered, and termination of therapy. Reveiwing Therapy

A 6 Session Review: during psychotherapy this provides an opportunity for you and I to evaluate the previous sessions, and if appropriate, to set/re-set goals and aspirations of what you would like to achieve from further therapy. During CBT therapy progress is reviewed for feedback on a regular basis as part of the CBT process.


Maintenance, Booster Sessions and Ending Therapy

Each person's needs are unique to them, and therapy is flexible to offer a variety of paths to suit different needs. Some people may want to finish sessions after completing a set of weekly sessions, whilst others may prefer to meet every 2 weeks to maintain the therapy work. Booster sessions are offered for CBT therapy at 1, 3 and 12 months for clients to review and refresh the skills they have learnt through therapy. Ending therapy is always the client's decision unless the therapist detects there could be a dependence upon therapy, this will be discussed and worked through together to form a plan to appropriately lead-up to ending sessions where the client feels supported and safe.

Ethics & Confidentiality

I am a fully-Accredited Member of the Irish and British Associations of Counselling and Psychotherapy (IACP) and (BACP) and abide by their Code of Ethics and Professional Practice (copies available on request). I am currently in the process of accreditation with the British Association for Cognitive Beahvioural Psychotherapists (BABCP).


Confidentiality

The session content and all relevant materials to the client’s treatment will be held confidential, unless the client requests in writing to have such content released to a specifically named person/persons. Any request is to made by email.

Everything discussed in counselling is confidential as laid down by the IACP, BACP and BABCP Codes of Ethics. There are some limitations to exclusive confidentiality which are legal requirements and are detailed below.


Limitations of Confidentiality:

1. If the Client or Therapist evaluates that the client is a danger to self – the therapist will contact Client’s Emergency Contact (please see request for emergency contacts below).


2. If the Client or Therapist evaluates that the client is a danger to others or to the therapist – Gardaí will be contacted.


3. Any disclosure of abuse involving a minor (under 18 years) or any other child protection concerns emerge, whether current or in the past where there is a child/children currently at risk – a mandatory report is made to Duty Social Worker in Tüsla, and they will decide if they will investigate this further or not.


Wherever possible, any limitation of confidentiality will be done with your knowledge, providing an extension of care through this process.

Client Notes and Clinical Supervision

I am required by IACP to regularly present my work in clinical supervision. All supervision is confidential and your name is never used or identified. Confidential working notes of sessions are required by the IACP to be kept, your personal information is not written on any working notes. The notes are a brief summary of the session to help the therapist track sessions.


GDPR and Client Records

The personal data that is collected on this form is only accessed by Justine Wilson (the therapist). This data is collected for identification details, emergency contact details in the only instance of risk to yourself, and to evaluate your general mental health wellbeing at the commencement of therapy. No one can access your personal data. The only other personnel who the data may be shared with, is identified above regarding 'limitations to confidentiality' such as your emergency contact, the Gardai or Tusla family agency if a risk to self, others or minors is identified through therapy. Under GDPR you are entitled to request the therapist to not retain client notes of your sessions, please advise the therapist if you wish to avail of this right. Written request is required from the client by email to to the therapist to share any client information with another party, this includes the client's records and also verbal conversations.
